Remarks - If Service Agreement is executed on behalf of the Shipper

by a person other than an Officer of the Company, documentation

confirming that person's authorization to execute contracts must be

provided at the time the Shipper returns Service Agreement.

If Shipper has not previously completed a Credit Application, a

Credit Application must be submitted.
