GENERAL TERMS AND CONDITIONS
2. QUALIFICATION FOR SERVICE
2.1 Requests for Service
Shippers requesting new or additional service under any rate
schedule must provide the information required by this
Subsection 2.1 and on Transporter's "Service Request Form"
(available on Transporter's Web Site). No service will be
scheduled for receipt and delivery until a completed "Service
Request Form" with respect to such service has been provided to
Transporter. A "Service Request Form" shall be tendered no
earlier than ninety days prior to the proposed commencement date
of service, unless the construction of new facilities is
required, unless the request is for capacity that will not be
available until the proposed commencement date or unless the
request is for capacity posted by Transporter pursuant to
Section 14 of the General Terms and Conditions of this tariff.

Any modification of an existing service shall be requested by
Shipper's submission of a new "Service Request Form" with a
notation on the Form that the service requested is a
modification of an existing service.
Any request shall include the following:
(a) Receipt/Delivery Points: The requested receipt and delivery
points as set forth in the applicable rate schedule,
(b) Quantities: The requested quantities, as set forth in the
applicable rate schedule.
(c) Term: The proposed commencement date and term of service.
