each such point;

(A) if more than one point, a

total for all such points;

(4) The name of the transporter(s) of

the gas to the Point(s) of Receipt;

(5) The name of the transporter(s) of

the gas from the Point(s) of

Delivery;

(6) The name, title, mailing address,

and telephone number of the

person designated by Shipper for

purpose of scheduling and

nominations under Sections 12

herein;

(7) The information, including mailing

address of Shipper, necessary for

billing under Section 6 herein.
