(2) Date the request was accepted as valid (to be left

blank by the Potential Shipper and filled in by

PGPL).

(3) The specific affiliation, if any, of the requester

with PGPL and the extent of the PGPL's

affiliation, if any with the person to be provided

transportation service.

(4) The extent of the supplier's affiliation with PGPL.

(5) (A) The complete legal name of the Shipper

making the request.

(B) The Shipper's designation (local distribution

company, interstate pipeline, intrastate

pipeline, end-user, producer or marketer).

(6) Maximum contract volume of gas requested to be

transported,

(A) Daily,

(B) Annually, and

(C) Over the life of the contract.

(7) The producing area of the source of the gas to be

transported.
