Each request shall contain:

(a) The name, business address and phone number of

the shipper requesting service and the party with

whom all contact should be made.

(b) A designation of whether Shipper is a local

distribution company, intrastate pipeline company,

interstate pipeline company, producer, end-user or

marketer

(c) A declaration of Shipper's specific affiliation

with Transporter, and the role of Transporter's

affiliate, if any, in the transportation transaction

(i.e., shipper, marketer, supplier, seller).

(d) The contract demand or daily volume of gas to

be transported.

(e) The initial term of the service, including

commencement and termination dates.
