and that Requesting Shipper's request will become part of a log available for

public inspection.

Shipper understands that it must reimburse OkTex Pipeline Company for the

Commission's filing fees upon receipt of an invoice therefor. Requesting

Shipper, by its signature, represents and certifies to OkTex Pipeline Company

that the information set forth in this Service Request Form is correct and

accurate and that the certifications set forth in paragraph (l) above have

been met.
