27. INFORMATION REQUIRED FOR A REQUEST FOR SERVICE.

The information below is required for Northern to process a request for

service and to tender a Service Agreement. If Northern does not have

all of the applicable information, Northern will work with the Shipper to

obtain it.

Base Information:

o Shipper's name.

o Certification for transportation under NGPA Section 311(a)(1)

executed by Shipper and by the "on behalf of" party. [if applicable]

o Eligible NGPA Section 311 "on behalf of" party (i.e., Intrastate,

Hinshaw or LDC). [if applicable]
