GENERAL TERMS AND CONDITIONS

25. REQUESTS FOR SERVICE

Subject to any conditions set forth in the applicable rate schedule, this Section

shall govern qualification for receipt of service from Company.

25.1 Requests for Service

All persons requesting service under any of Company's rate schedules or persons

requesting Operational Balancing Agreements must provide, via the System or by

fax or in writing when necessary, the information required by this Section 25

and all information necessary to fill in the blanks and complete the applicable

form of agreement in order to qualify for service. No request for service will

be scheduled until all of the required information has been provided.
