(f) Requested Point(s) of Receipt and Delivery and the maximum

quantities to be received by Transporter by receipt point and to be delivered

to Shipper by delivery point;

(g) Names of entities who will deliver gas to Transporter and

entities who will receive gas from Transporter;

(h) Term of Service, including (a) the date service is requested to

commence and (b) the date service is requested to terminate;

(i) A statement that Shipper has title to the gas to be transported

or the right to acquire title to such gas and has entered into or will enter

into all necessary arrangements to ensure that all upstream and downstream

transportation will be in place prior to commencement of service; and

(j) Documentation of Shipper's credit worthiness, as provided in

Section 8.5.
