8. OTHER CONDITIONS OF SERVICE

8.1 Requests for transportation hereunder shall be considered complete and

acceptable only if the following information is provided in writing and

sent to the Transporter via facsimile, e-mail, courier or U.S. Mail at

the following address:

(a) Shipper's Name and Address;

(b) Identity of Shipper (including class of Shipper, e.g., Local

Distribution Company, Intrastate Pipeline, Hinshaw Pipeline, Interstate

Pipelines, Marketer, Producer, End-User, Other);

(c) Type of Service Requested (firm or interruptible);

(d) Any affiliation of the requester with Transporter;

(e) Maximum Daily Quantity to be transported during the requested term

of service by Transporter;
