(e) Maximum Daily Quantity to be transported during the requested

term of service by Transporter;

(f) Requested Point(s) of Receipt and Delivery and the maximum

quantities to be received by Transporter by receipt point and

to be delivered to Shipper by delivery point;

(g) Names of entities who will deliver gas to Transporter and

entities who will receive gas from Transporter;

h) Term of Service, including (a) the date service is requested

to commence and (b) the date service is requested to

terminate;
