4. REQUESTS FOR SERVICE

4.1 REQUEST FOR SERVICE FORM - Each Shipper requesting Firm

or Interruptible Transportation service hereunder shall

provide the information specified in the Request for

Service form, the form of which is included in this FERC

Gas Tariff in the "Forms" section. Each request must be

accompanied by a Prepayment of Ten Thousand Dollars

($10,000).

4.2 INCOMPLETE SERVICE FORMS - Request for Service forms

received by Transporter that do not include the

Prepayment required by Subsection 4.1., or do not contain

all the requested information, including credit

information sufficient to demonstrate that a Shipper will

be able to meet its financial obligations under the

requested Service Agreement, will be considered

incomplete. Transporter will notify Shipper of any

deficiencies and Shipper's request for service will not

be considered until all of the required information is

provided to Transporter. The request for service will be

deemed invalid if Shipper fails to provide the required

information within ten (10) Business Days after

notification by Transporter that the request is

incomplete.
