THIS AGREEMENT, made and entered into as of the ____ day of _____________, 20___, by and between

COLUMBIA GAS TRANSMISSION, LLC, a Delaware limited liability company (hereinafter called Transporter), and

___________________________________________________, a ________________________ corporation (hereinafter called

Shipper):

W I T N E S S E T H:

WHEREAS, the parties hereto have executed a Service Agreement dated _____________

_______________________ wherein Transporter has agreed to sell and deliver and Shipper has agreed to purchase

and receive certain quantities of natural gas ________________________________________________.

NOW, THEREFORE, in consideration of the mutual covenants herein contained, the parties hereto agree as

follows:
