transported during the period for which

transportation is requested.

(i) The producing area of the source of the gas

requested to be transported

(j) Term of Service

(1) Date service requested to commence; and

(2) Date service requested to terminate.

(k) Receipt Point(s)

(1) The Receipt Point(s) into Chandeleur's

system; and

(2) The name of the pipeline, gatherer,

producer, or other entity delivering the

gas into Chandeleur's system.

(l) Delivery Point(s)

(1) Point(s) of delivery by Chandeleur; and

(2) The name of the pipeline or other entity

receiving the Gas downstream of

Chandeleur.

(m) The state of the ultimate end-user of the

gas.

(n) The identity of Chandeleur rate schedules and

the transportation rates applicable for such

service.
