Applicant must provide current audited financial

statements, annual reports, 10-K and other similar

regulatory filings and reports.

On behalf of Applicant, the undersigned, who is

duly qualified to act on behalf of Applicant,

agrees to the following terms.

1. Applicant authorizes its vendors, banking institutions

and financing entities to release its account history

to CEGT for the purpose of credit evaluation.

2. Applicant understands that remittances for all

purchases must be made with immediately available

funds, unless other arrangements have been agreed to

by CEGT in writing.

3. Applicant must remit for purchases at the appropriate

address provided from time to time by CEGT and in

accordance with the terms and conditions shown on each

invoice.
